- The practice is hiring a Family OR Adult Geriatric Nurse Practitioner.
- Experienced providers and new grads are welcome to apply.
- Start date will be 120 days after having a signed offer to allow for credentialing lead time.
- Work hours Monday-Friday 8:00-5:00.
- 100% outpatient care with no hospital responsibility.
- After-hours call coverage is shared among 9 providers - 3 to 4 days every 4th week.
- Average 18-20 patients per day after ramp up period.
- Patient panel is age 16 and up - adult/geriatric - but open to all ages.
- Support and Ancillary services: Point of Care Testing, in-office Lab, X-ray Tech, PharmD to assist with patient medication and assistance program.
- All our providers have 1:1 dedicated support from CMA/LPN.
- EPIC EMR and MyChart for telehealth.
- Looking for a mature Nurse Practitioner that fosters relationships with their patients and who is able to work collaboratively in a group setting. Our team is very outcomes focused and places a high priority on patient experience of care. Provider must be proficient at health maintenance, managing diabetes and hypertension, women's health and Medicare Annual Wellness visits.
